Licensed Practical Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse to join our hospice care team. As a palliative care specialist, you will provide high-quality, patient-focused care to terminally ill patients and their families.
Your primary responsibility will be to assess the needs of patients, coordinate interdisciplinary plans of care, and implement nursing interventions to ensure optimal quality of life.

Responsibilities- Assess patient needs and develop individualized plans of care
- Coordinate with interdisciplinary teams to ensure comprehensive care
- Implement nursing interventions to promote patient comfort and well-being
- Evaluate care outcomes and make revisions as needed
- Graduate of an accredited school of nursing
- Current LPN license to practice in the state of Indiana
- CPR certification
- Infusion skills preferred
