Position Overview:
This role is integral to the Families and Communities Together (FCT) initiative, aimed at empowering families and enhancing support systems for youth facing significant emotional challenges. The Case Manager will operate within a multidisciplinary framework, ensuring that youth with Serious Emotional Disturbance (SED) receive comprehensive care tailored to their unique needs.


Key Responsibilities:

  • Exhibit a foundational understanding of and commitment to the CBHI principles: Child-Centered, Family-Driven, Strength-Based, Culturally Responsive, Collaborative, and Committed to Continuous Quality Improvement.
  • Manage a diverse caseload encompassing Therapeutic Mentoring (TM), In-Home Therapy (IHT), and Intensive Care Coordination (ICC).
  • Conduct thorough home-based clinical evaluations, including risk assessments and treatment planning.
  • Facilitate the assembly of Care Planning Teams, incorporating family members and relevant support systems.
  • Lead regular meetings to develop personalized Care Plans for each youth and family.
  • Collaborate closely with the FCT Family Partner and other team members to monitor and adjust Care Plans as necessary.
  • Engage in ongoing communication with other service providers to ensure cohesive treatment.
  • Deliver weekly therapeutic sessions in home settings, focusing on enhancing family dynamics.
  • Provide mentoring sessions aimed at skill development for youth.
  • Maintain comprehensive documentation of client interactions, assessments, and treatment plans.
  • Adhere to program productivity standards and participate in required training and meetings.
  • Travel to various community settings and family homes as part of service delivery.
  • Participate in an on-call rotation to provide telephonic support to families.

Qualifications:

  • Bachelor's degree in a relevant human services field (e.g., social work, psychology, counseling).
  • A minimum of two years of experience in a clinical environment, particularly with children and families facing behavioral and emotional challenges.
  • Proven ability to engage effectively with youth and families, advocating for their needs within a collaborative framework.
  • Comfortable delivering services in both home and community settings.
  • Strong work ethic, independent initiative, and a commitment to team objectives.
  • Excellent communication and documentation skills.
  • Valid US driver's license and access to a registered and insured vehicle for work-related travel.
