This RN Supervisor position involves regular patient visits in the designated service area. Candidates who meet the qualifications will be offered a competitive salary along with comprehensive benefits. Proficiency in Spanish is a mandatory requirement.

Key Responsibilities:

  • Take charge of coordinating, implementing, and continuously assessing the home care requirements of patients.
  • Review patient information and ensure that critical findings from assessments are communicated effectively to both family members and professional caregivers.
  • Exhibit strong customer service, communication, documentation, and time management skills to ensure success in this role.

Qualifications:

  • Exceptional clinical assessment abilities.
  • Proven problem-solving skills and the capacity to collaborate with an integrated team.
  • Valid New York State RN license and registration.
  • A minimum of one year of RN experience in Med/Surg, Hospital, or Home Healthcare settings is preferred.
  • Prior experience in a home care environment is advantageous.
  • A reliable vehicle is necessary for patient visits.
  • Bilingual Spanish is required.
