Job Description

The Social Worker is a vital member of our healthcare team, responsible for delivering high-quality social services to our patients. This includes conducting psychosocial evaluations, providing treatment and advocacy, teaching, and making referrals to community services as needed. The Social Worker will also assist with discharge planning, ensuring that patients receive the necessary support and resources to maintain their health and well-being.

Key Responsibilities:
  • Conduct interviews with patients and their significant others to gather information about their social, emotional, and family situation.
  • Communicate social and family information to attending physicians and other patient care team members.
  • Enable patients and their significant others to understand, accept, and follow medical recommendations through individual or group conferences.
  • Determine social assessments for discharge planning and intervene appropriately in instances of abuse, assault, or neglect.
  • Make appropriate referrals to resolve patient issues effectively.
  • Participate as a team member in conferences on behalf of patients with other healthcare personnel and community agencies.
  • Contact community resources and other pertinent sources to arrange services on behalf of patients.
Qualifications:
  • Bachelor's Degree in Social Work (BSW) or related degree in human services.
  • Computer proficiency and literacy.
  • Knowledge of CMS, Medicare/Medi-Cal, and Joint Commission requirements.
  • Excellent written and verbal communication skills in English.
  • Ability to establish and maintain effective working relationships across the organization.
  • Ability to facilitate and lead interdisciplinary rounds.
Preferred Qualifications:
  • Master's Degree in Social Work or related degree in human services.
  • One year of healthcare experience.
  • Bilingual skills to communicate effectively with patients and families.

Pay Rate: Min - $37.00 | Max - $37.00
