The OpportunityWe're seeking a highly skilled Nurse Practitioner or Physician Assistant to join our outpatient practice in Lawrenceville, Georgia. As a key member of our care team, you'll work closely with our medical director to provide high-quality patient care and contribute to ongoing practice innovation.

What You'll Do- Provide best-in-class comprehensive primary care in an outpatient setting.
- Partner with our care team to leverage our coaching and integrated behavioral health models to engage our geriatric patient population in care and drive behavior change.
- Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population.
- Enjoy weekly protected time for team building and shared discussion of challenging cases.
- At least 2 years of primary care experience.
- Current licensure in the state of Georgia.
- Excellent communication and relationship-building skills.
- Ability to thrive in a fast-growing, mission-driven organization.
