Job Overview

We are seeking an advanced practice clinician to join our team. The ideal candidate will have experience working in long-term care settings and possess strong clinical skills.

Key Responsibilities:
  • Provide high-quality patient care by assessing and diagnosing patient conditions.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to promote patient-centered care.
Qualifications:
  • Current, unrestricted valid Nurse Practitioner license from the Board of Nursing/PA License from the Medical Board in the state of assignment or the ability to obtain.
  • DEA certificate current or the ability to obtain.
  • Clinical background in adult, family, or geriatric specialties preferred.
  • Past experience working in long-term care, a nursing home, or with seniors in other settings preferred.
  • Strong clinical and communication skills preferred.
