About the Role

We are seeking a dedicated and compassionate Advanced Practice Clinician to join our team at Liberty Health. This role involves providing exceptional care to patients in skilled nursing facilities, ensuring their physical and emotional well-being.

Key Responsibilities
  • Conduct thorough assessments of patients to identify their healthcare needs.
  • Develop and implement personalized care plans to meet the unique requirements of each patient.
  • Collaborate with physicians, nurses, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Maintain accurate and detailed records of patient interactions, treatment plans, and outcomes.
Requirements
  • A valid Nurse Practitioner license from the Board of Nursing or PA License from the Medical Board in the state assigned or ability to obtain.
  • Current DEA certificate or ability to obtain.
  • Clinical experience in adult, family, or geriatric specialties preferred.
  • Prior experience working in long-term care settings is an advantage.
  • Strong communication and interpersonal skills are essential.
  • Proficiency in PC-based word processing software, including Microsoft Word and Outlook.

  • A competitive salary range estimated to be $95,000 - $115,000 per year, depending on experience.
