About the Job

As a Home Health Aide with SolutionHealth, you will play a vital role in providing high-quality care to patients in their homes. Your primary responsibility will be to perform delegated nursing, homemaking, and other delegated duties for patients of all ages and families in their home or other designated facility.

Key Responsibilities
  • Report and document patient/client needs and responses to the appropriate Case Manager and/or Clinical Manager.
  • Report patient/client changes in condition or situations immediately.
  • Adhere to patient/client Home Health Aide care plan as set up by the primary nurse/therapist while maintaining a healthy, safe environment.
  • Reinforce the teaching of the primary nurse/therapist by reviewing personal care techniques with caregivers and patients.
  • Complete documentation in clinical records, time sheets, and quality control logs.
  • Attend a minimum of 12 hours of in-services per year.
  • Provide care while promoting client/patient dignity and providing a calm, supportive, and therapeutic environment for clients/patients and their families.
Requirements
  • High school diploma or equivalent preferred.
  • Six (6) months to one (1) year of experience in a related field desired.
  • Current Nursing Assistant License in the State of New Hampshire required.
  • Driver's License required to travel throughout New Hampshire to provide home health care services and other job-related activities.
