About the Job

We are seeking a skilled Home Health Aide to join our team at SolutionHealth. As a Home Health Aide, you will provide delegated nursing, homemaking, and other delegated duties for patients of all ages and families in their home or other designated facility.

Responsibilities include:

  • Reporting and documenting patient/client needs and responses to the appropriate Case Manager and/or Clinical Manager.
  • Reporting patient/client change in condition or situations immediately.
  • Adhering to patient/client Home Health Aide care plan as set up by the primary nurse/therapist while maintaining a healthy, safe environment.
  • Reinforcing the teaching of the primary nurse/therapist by reviewing personal care techniques with caregivers and patients.
  • Completing documentation in clinical records, time sheets, and quality control logs.
  • Attending a minimum of 12 hours of in-services per year.
  • Providing care while promoting client/patient dignity and providing a calm, supportive, and therapeutic environment for clients/patients and their families.
Requirements

To be successful in this role, you will need:

  • A High school diploma or equivalent preferred.
  • Six (6) months to one (1) year of experience in a related field desired.
  • A Current Nursing Assistant License in the State of New Hampshire required.
