Responsibilities:

As a Registered Nurse, you will be responsible for:

  • Following a designated care plan in accordance with patient needs
  • Performing home visits to patients in Springfield, Holyoke, Westfield, Chicopee, and surrounding towns
  • Administering medication, wound care, treatments, and procedures
  • Monitoring clients' conditions and reporting changes to the RN Clinical Manager
  • Following up with, executing, and properly documenting doctors' orders
  • Performing client assessments as necessary
  • RN case management and coordination
  • Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet
Qualifications:

To be considered for this role, you must have:

  • A current license as a Registered Nurse in Massachusetts
  • Graduation from an accredited and approved nursing program
