Job Summary

As a Licensed Practical Nurse (LPN) at Third Street Family Health Services, you will play a vital role in delivering high-quality patient care in our health clinic. Working under the supervision of registered nurses and physicians, you will assist in the coordination and delivery of patient care services, ensuring patient comfort, safety, and well-being while promoting efficient clinic operations.

Key Responsibilities
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards in accordance with the Ohio Nursing Practice ethical and professional guidelines.
  • Accurately document pertinent observations and interventions in the Electronic Medical Record to aid in the development of the patient-centered care plan and the delivery of quality and equitable health care.
  • Carry out physician orders timely and accurately.
  • Maintain quality control standards, logs, and point-of-care testing.
  • Attend required Clinical and Health Center in-service programs.
  • Demonstrate responsibility and accountability for professional practice, continued growth, and self-evaluation.
  • Comply with the Third Street Family Health Services infection control policies and procedures.
  • Understand and demonstrate compliance with Health Services and Resources Administration (HRSA) and Third Street Family Health requirements.
  • Maintain appropriate licensure and participate in continuing education activities to exemplify the Licensed Practical Nurse.
  • Ensure and uphold the confidentiality requirements of all patient records, and manage all daily tasks and activities consistent with HIPAA, state, and federal laws and regulations, as well as the clinic's policies and regulations regarding confidentiality and security.
  • Provide culturally competent quality patient-centered health care in partnership with the communities we serve.
  • Provides clinical and administrative support and functions as a resource and mentor for new staff and students assigned to their work site.
  • Assists the Clinical Nursing Manager in ensuring that nursing staff follow the Nursing Practice Act in their delivery of comprehensive patient care with maintenance of strict confidentiality.
  • Ability to develop trust and effective working relationships with staff members, individuals, families, and external and internal professionals.
  • Strong information gathering/interviewing techniques.
  • Be a positive role model for all health center staff.
