Job Overview

AccentCare, Inc. is seeking a skilled Clinical Support Specialist LPN to join our team of dedicated healthcare professionals. As a Clinical Support Specialist LPN, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Key Responsibilities:

  • Provide clinical support to patients and families in their homes, promoting independence and self-care.
  • Collaborate with healthcare teams to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patient progress and adjust care plans as needed.

Requirements:

  • RN or LVN/LPN licensure to practice nursing in the state of employment.
  • CPR certification.
  • Previous experience in home care settings preferred.
  • Prior use of Electronic Medical Records (EMR) preferred.
  • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
  • Demonstrates excellent observation, nursing judgment, and communication skills.
  • Must be a licensed driver with an automobile in good working order.
