Job Summary

The Home Health RN will be responsible for managing and delivering comprehensive home health services, including assessments, interventions, and supportive care to clients within their place of residence. This role requires strong communication and interpersonal skills, as well as the ability to work autonomously and make sound clinical judgments.

Key Responsibilities

  • Assess patients' physical, psychosocial needs in a sensitive and caring manner, following established Standards of Nursing Practice and VNA procedures.
  • Develop and implement individualized care plans, in collaboration with patients, families, and healthcare providers.
  • Provide education and support to patients and families, promoting self-care and independence.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
  • Document patient information accurately and timely, in accordance with agency policies and regulatory requirements.

Requirements

  • Massachusetts RN licensure.
  • Valid state-issued driver's license.
  • One year of acute medical/surgical nursing experience.
  • CPR certification.
