Overview

Enhabit Home Health & Hospice is seeking a skilled RN Clinical Lead to join our team. As a key member of our leadership team, you will play a critical role in ensuring the delivery of high-quality home health services to our patients.

The RN Clinical Lead will be responsible for coordinating home health services, reviewing patient information, and providing oversight of patient care. This role requires strong leadership and communication skills, as well as the ability to work collaboratively with our clinical team.

Responsibilities

Coordinate home health services to ensure timely and effective delivery of care

Review and approve patient information, including start of care, recertification, and resumption of care

Provide oversight of patient care, including case conferences and order reviews

Assist with day-to-day office and staff management related to patient care

Provide ongoing education and training to clinical staff on documentation requirements and regulatory standards

Qualifications

Graduate of an accredited school of professional nursing and licensed in the state of employment

A minimum of two years nursing experience and one year experience in home health

Strong leadership and communication skills

Ability to work collaboratively with clinical team
