Overview:
Seasoned Recruitment is in search of a dedicated and skilled Geriatric Care Nurse Practitioner to become an integral part of our interdisciplinary team (IDT). This role involves providing direct medical services to participants in the PACE (Program of All-Inclusive Care for the Elderly) program. As a Geriatric Care Nurse Practitioner, you will collaborate with the IDT to develop and implement care strategies aimed at improving the health and well-being of our participants.
Key Responsibilities:
- Conduct thorough in-person evaluations, including initial assessments, semi-annual reviews, and unscheduled evaluations, ensuring continuous medical support for participants.
- Collaborate with physicians and the Interdisciplinary Team (IDT) to create tailored care plans for each participant.
- Engage with physicians and the IDT in planning discharges from both acute and long-term care environments.
- Oversee the care of participants residing in nursing facilities, making regular visits as needed based on individual requirements.
- Work in partnership with physicians, community-based PACE doctors, and the IDT to ensure effective coordination of care.
- Under the guidance of the Medical Director, analyze outcomes and utilization data, including hospital admissions and specialist referrals, to identify best practices and enhance policy frameworks.
- Participate in scheduled night and weekend on-call duties.
Qualifications:
- Graduation from an accredited nursing program.
- Certification as a Nurse Practitioner with Prescriptive Authority.
- Active RN State License.
- A minimum of two years of experience working with elderly populations.
- Current understanding of chronic care and geriatric best practices.
- Access to reliable transportation.
