Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient.
- Provide hands-on care, including medication administration, wound care, and vital sign monitoring.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
- Document patient progress and outcomes to ensure accurate and timely reporting.
- Current Registered Nurse (RN) license for the state of Idaho.
- Current Health Certificate and PPD or Chest X-Ray.
- Current BLS card.
- One year prior RN experience preferred.
