About the Role

As a RN Clinical Liaison, you will be responsible for educating healthcare professionals about our hospice services and providing end-of-life solutions that prioritize clinical excellence, compassion, and dignity.

Key Responsibilities
  • Ensure effective and timely intake and referral processes to meet patient, family, and referral source needs
  • Complete admissions processes for emergency cases and inpatient hospice admissions
  • Provide accurate and timely information about hospice services to inquirers
  • Respond to referral source calls, initiate paperwork, and begin the intake/referral process
  • Serve as a patient advocate in evaluating and developing plans of care and obtaining informed consent for hospice
  • Call on multiple account types, including SNFs, ALFs, hospitals, and physicians
  • Offer in-services and education on the benefits of hospice
Requirements

To be successful in this role, you will need:

  • A RN license in the state(s) you will be employed
  • Minimum of 2 years of healthcare experience in a setting such as hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator
  • Known and respected within your community
