Job Summary

SSM Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Long-Term Care setting. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our patients.

Key Responsibilities
  • Assessment and Care Planning: Contribute to the assessment of patients and develop individualized care plans to ensure the well-being of patients.
  • Medication Administration: Administer medications and monitor patients for adverse reactions to medications or treatments.
  • Lab Tests and Therapies: Perform laboratory tests and therapeutic services as needed.
  • Patient Education: Educate patients and their families about disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care.
  • Work Environment: Work in a safe and alert manner, adhering to all safety protocols and procedures.
Requirements
  • Education: Graduate of an accredited school of nursing.
  • Experience: No experience required, but a strong desire to provide exceptional patient care is essential.
Physical Demands
  • Constant Use: Constant use of speech to share information through oral communication.
  • Constant Standing and Walking: Constant standing and walking throughout the workday.
  • Frequent Lifting and Carrying: Frequent lifting and carrying of objects weighing 0-25 lbs.
  • Frequent Sitting and Keyboard Use: Frequent sitting and use of a keyboard for data entry.
  • Frequent Use of Vision and Depth Perception: Frequent use of vision and depth perception for distances near and far.
  • Frequent Use of Smell: Frequent use of smell to detect and recognize odors.
  • Frequent Use of Hearing: Frequent use of hearing to receive oral communication and distinguish body sounds.
Licenses and Certifications
  • Basic Life Support (BLS) Certification: Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • Licensed Practical Nurse (LPN) License: Licensed Practical Nurse (LPN) license in the state of employment or a compact multi-state license.
