Job Summary

SSM Health is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at St. Mary's Care Center. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the well-being of our residents.

Responsibilities
  • Assessment and Care Planning: Contribute to the assessment of patients and develop individualized care plans to ensure the well-being of our residents.
  • Medication Administration: Administer medications and monitor patients for adverse reactions, ensuring the safe and effective delivery of care.
  • Lab Tests and Therapeutic Services: Perform laboratory tests and provide therapeutic services as needed, working closely with our interdisciplinary team.
  • Patient Education: Educate patients and their families about disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care.
  • Communication and Teamwork: Maintain open and effective communication with patients, families, and healthcare team members, ensuring seamless care coordination and collaboration.
Requirements
  • Education: Graduate of an accredited school of nursing.
  • Experience: No experience required, but a strong desire to provide exceptional patient care and contribute to a dynamic healthcare team.
  • Licenses and Certifications: Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA), Licensed Practical Nurse (LPN) - State of Work Location or Compact Multi-State License.
