We are seeking a highly skilled and motivated Licensed Practical Nurse (LPN) to join our healthcare team in an SNF setting at Interim Healthcare of Bellevue, WA.
As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care, adhering to established policies and procedures, and collaborating with the healthcare team to implement nursing care plans.
Key Responsibilities:- Ensure adherence to Universal Precautions for resident and staff safety, promoting a culture of safety within our facility.
- Prepare residents' medications for short-term absences, following state regulations and guidelines.
- Collaborate with the healthcare team to develop and implement individualized care plans, ensuring seamless coordination of care.
- Initiate preventive and rehabilitative nursing procedures as appropriate, addressing the unique needs of each patient.
- Observe signs and symptoms, reporting them to the physician and RN, and taking proactive steps to address any changes in the patient's physical or emotional condition.
- Teach and counsel patients and their families/significant others on nursing care needs and related issues, empowering them to take an active role in their care.
- Participate in medical record audits, ensuring accuracy and completeness of patient records.
- Attend staff meetings, patient care conferences, and in-services, staying informed about best practices and new developments in the field.
- Read Daily Communication Logs for resident information, staying up-to-date on important details and updates.
- Document any changes in resident status in Progress Notes, maintaining accurate and detailed records.
- Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in WA state.
- Active BLS certification, demonstrating a commitment to emergency preparedness and response.
- Preferred minimum of one year of nursing experience, with a strong track record of delivering high-quality patient care.
- Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and colleagues.
- Ability to work well in a fast-paced SNF environment, adapting to changing situations and priorities.
- Two positive work references, verifying your professional credentials and reputation.
- Successfully complete and pass a background check and drug test, meeting the highest standards of integrity and reliability.
