Overview

Responsibilities
  • Provide comprehensive nursing care to patients with mental health conditions, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and support.
  • Conduct regular home visits to assess patient progress, provide education, and address any concerns or issues that may arise.
  • Document patient information, including progress notes, medication lists, and other relevant data, in a timely and accurate manner.
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One year of recent nursing experience, recommended to be within the last 3 years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, a psychiatric Partial Hospitalization Program (PHP), or another out-patient psychiatric service.
  • Valid CPR certification and a licensed driver with an automobile that is insured in accordance with state and/or organization requirements.
