Job Summary

We are seeking a highly skilled and compassionate Case Manager III to join our team at St. Vincent de Paul CARES. As a key member of our Shelter Services department, you will play a vital role in providing assessment, monitoring, planning, linkage, and advocacy for individuals and families enrolled in our rapid rehousing services.

Key Responsibilities
  • Conduct thorough assessments of individuals and families to determine eligibility and develop personalized housing stability plans.
  • Monitor progress and provide ongoing support to ensure successful housing outcomes.
  • Collaborate with community resources and service providers to connect clients with necessary services and support.
  • Develop and maintain accurate documentation of client progress and services provided.
  • Work closely with the Coordinated Entry System to ensure seamless service delivery.
Requirements
  • Bachelor's degree in social work or related field.
  • Minimum 2 years of experience serving homeless or at-risk populations.
  • Strong understanding of homelessness, severe and persistent mental illness, and substance abuse.
  • Knowledge of resources in the community available for the homeless population.
  • Ability to form partnerships in the community and seek out community resources.
