About the Role

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Lifeline Health Care of Fayette. As a key member of our home health care team, you will play a vital role in providing high-quality patient care and support to our patients in the comfort of their own homes.

Responsibilities
  • Provide skilled nursing care to patients in their homes, as defined by state laws and in accordance with the plan of care.
  • Conduct thorough assessments of patients' nursing needs and develop individualized care plans to address these needs.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Current LPN licensure in the state of Kentucky.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
