Responsibilities- Provide skilled nursing care to patients in their homes, as defined by state laws and in accordance with the plan of care.
- Conduct thorough assessments of patients' nursing needs and develop individualized care plans to address these needs.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Current LPN licensure in the state of Kentucky.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
