The Licensed Practical Nurse (LPN) plays a vital role in delivering high-quality care to patients in a memory care setting. This position involves administering medications and providing essential support to patients and their families.
Key Responsibilities
- Assist in evaluating patients' physical, psychosocial, environmental, spiritual, social, and educational needs, documenting responses to nursing interventions while collaborating with the healthcare team to achieve optimal patient outcomes.
- Contribute to the development and execution of care plans, including discharge planning, adhering to nursing standards and health center policies, while addressing patient and family needs through critical thinking.
- Facilitate educational opportunities for patients and families based on the teaching plans established by registered nurses.
- Adapt to the dynamic healthcare environment by embracing technological advancements and process changes, demonstrating a willingness to learn and maintaining composure under pressure.
- Effectively communicate significant patient changes to registered nurses and other multidisciplinary team members.
- Participate in departmental meetings, in-services, and educational sessions, with an attendance requirement of 80%, including 50% in-person participation when applicable.
- Act as a steward for hospital resources, meeting productivity expectations as defined by unit management, and ensuring the appropriate use of supplies.
- Engage in process improvement initiatives that enhance patient satisfaction, clinical outcomes, and financial stability.
- Document patient information accurately and promptly in electronic and paper medical records.
- Delegate tasks appropriately based on patient needs and scope of practice.
- Identify personal learning and development needs, setting yearly performance goals and creating a plan for professional growth.
- Maintain confidentiality in accordance with HIPAA regulations.
- Provide competent care to designated patient populations by accurately interpreting data and adapting procedures based on age-specific growth and development.
- Enhance job performance through continuous personal and professional development, participating in relevant educational programs.
- Communicate and perform duties in alignment with the mission and values of the organization.
- Adhere to all medical center policies and safety procedures, proactively identifying safety concerns in the workplace.
- Perform additional responsibilities as assigned by supervisors or department heads.
