We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team as a Hospice Nurse - Crisis Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and their families in need of end-of-life support.
Responsibilities- Coordinate patient care under the direction of the primary physician and registered nurse.
- Identify patient and family needs, and coordinate care accordingly.
- Contribute to the development and implementation of individualized plans of care as part of the interdisciplinary group.
- Utilize monitoring, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
- Pledge to prioritize patient comfort and care above all else.
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- 30 hour IV Certification preferred.
- Hospice Certification preferred.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
- Bilingual (English/Spanish) preferred.
