Responsibilities
  • Manage and supervise Home Health Aides, providing support, guidance, and mentorship to ensure quality patient care and compliance with company policies and procedures.
  • Conduct regular performance evaluations of Home Health Aides and provide feedback for professional development.
  • Oversee and coordinate the scheduling, assignment, and completion of patient care services.
  • Collaborate with other healthcare professionals to develop individualized treatment plans for clients.
  • Monitor and review client charts to ensure documentation and care plans align with regulatory requirements and best practices.
  • Participate in orientation and training of new nurses to ensure they understand and follow company policies and procedures.
  • Conduct regular nursing visits and make recommendations for improvement.
  • Act as a liaison between clients, families, and healthcare providers to ensure overall satisfaction and quality of care.
  • Develop and maintain relationships with community resources and referral sources.
  • Stay informed about changes and updates in the healthcare industry, specifically in home care, and implement necessary changes to maintain compliance.
Requirements
  • Valid RN license in the state of New Jersey.
  • Minimum of 3 years of experience in home care or related healthcare setting.
  • Knowledge of applicable state and federal regulations for home care.
  • Excellent communication, leadership, and problem-solving skills.
  • Ability to work independently and make critical decisions.
  • Proficient in electronic medical records and Microsoft Office.
  • Valid driver's license and reliable transportation.
