Job Summary

We are seeking a skilled ICU/PCU Nurse Navigator and Bed Manager to join our team at Trinity Health. This role will be responsible for providing quality patient-centered care, facilitating patient movement from the ED to inpatient units, and managing bed assignments.

Key Responsibilities
  • Provide quality patient-centered care, considering age-specific, developmental, cultural, and spiritual needs through competent clinical practice and application of the nursing process.
  • Facilitate patient movement from the ED to inpatient units, ensuring efficient and effective bed management.
  • Manage bed assignments, prioritizing patient needs and ensuring optimal bed utilization.
  • Collaborate with interdisciplinary teams to achieve patient outcomes, participating in rounds and handoffs as needed.
  • Identify and address patient and family education needs, developing and implementing teaching plans to ensure readiness for discharge.
  • Participate in quality improvement and evidence-based practice activities, integrating resultant changes into practice.
  • Maintain confidentiality of patient and hospital information, adhering to Trinity Health policies and procedures.
Requirements
  • Graduation from an accredited nursing program and current nursing license in the state where working.
  • Active BLS certification and required credentials for specialty area (e.g., ACLS, TNCC).
  • Minimum of two years of recent clinical experience in the area in which you work.
  • Knowledge of nursing theory, practice, and age-specific needs, as demonstrated by successful completion of introductory orientation behaviors for Registered Nurses.
  • Interpersonal skills necessary to initiate and maintain collegial relationships with coworkers and therapeutic relationships with patients and families.
  • Critical thinking skills necessary to perform principal duties and responsibilities of the job description.

This role requires the ability to walk and stand continuously, move, lift, and position patients, and work in a fast-paced environment with diverse personalities and work styles. The successful candidate will possess excellent verbal and written communication skills, be comfortable operating in a collaborative, shared leadership environment, and demonstrate a personal presence characterized by honesty, integrity, and caring.
