We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our team at Phoenix Home Care and Hospice. As a Pediatric In-Home Private Duty Nurse, you will provide high-quality, individualized care to children and young adults with specialized healthcare needs in the comfort of their own homes.
Key Responsibilities- Deliver compassionate, patient-centered care to pediatric patients based on their unique needs and treatment plans.
- Assist in the development, review, and revision of patient care plans.
- Build strong relationships with patients and their families to provide emotional support and reassurance.
- Monitor and record patient reactions to treatment and report any changes in condition to appropriate personnel and/or physicians.
- Provide ongoing education and supervision to patients, families, and other healthcare team members.
- Document patient care services via electronic charting and participate in quality assessment activities.
- Active LPN or RN license.
