Job Summary

Key Responsibilities
  • Case Management: Receive and review case referrals, assign clinicians to cases, and ensure that patient care is coordinated and managed effectively.
  • Clinical Oversight: Review and evaluate each case, provide guidance and support to clinicians, and ensure that care and services are delivered in accordance with organizational policies and procedures.
  • Team Leadership: Lead and manage a team of clinicians, provide coaching and development opportunities, and ensure that team members are equipped to deliver high-quality patient care.
  • Quality Improvement: Identify areas for quality improvement, develop and implement strategies to address these areas, and ensure that quality standards are met.
  • Compliance: Ensure that all organizational policies and procedures are followed, and that compliance with regulatory requirements is maintained.
Requirements
  • Education: Registered nurse with a current licensure to practice professional nursing in the state.
  • Experience: Two years of management or supervisory experience in a healthcare setting, preferably home care.
  • Skills: Excellent observation, organization, and verbal and written communication skills, proficiency in computers, Microsoft, EMR, and technology, and knowledge of governmental regulations and ACHC standards.
