RN Hospice Case Manager

Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Case Manager. As a key member of our care team, you will be responsible for providing patient-centered care, collaborating with the care team to develop and implement a plan of care, and making referrals to other hospice care disciplines as needed.
Responsibilities- Perform patient assessments and develop individualized patient care progress records with accuracy and timeliness.
- Collaborate with the care team to develop and implement a plan of care that meets the unique needs of each patient.
- Make referrals to other hospice care disciplines as needed, or document rationale for not doing so.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with clinical manager and care team to ensure seamless care coordination.
- Perform on-call responsibilities and provide on-call services to patients/families as assigned.
- One year of nursing experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Attractive pay, with a salary range of $92,000 to $100,000.
