- Conduct regular assessments and evaluations to identify changes in resident condition and adjust care plans accordingly.
- Document all patient interactions, including progress notes, medication administration records, and other relevant information.
- Participate in quality improvement initiatives, including data collection, analysis, and implementation of best practices.
- Maintain accurate and up-to-date records, including medical charts, medication lists, and other relevant documents.
- Communicate effectively with residents, families, and healthcare team members, ensuring clear and timely information exchange.
- Perform other duties as assigned, including participating in staff meetings, attending in-services, and contributing to facility-wide initiatives.
- Current licensure as a Licensed Practical Nurse in the State of Ohio.
- Ability to work effectively in a fast-paced, dynamic environment, prioritizing tasks and managing multiple responsibilities.
- Strong communication and interpersonal skills, with the ability to work collaboratively with diverse groups.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Flexibility to work varied shifts, including evenings, weekends, and holidays, as needed.
