Job Summary

The Licensed Practical Nurse provides direct care to residents in a long-term care setting, administering treatments and medications, and organizing daily assignments for direct care staff. In the absence of the Registered Nurse, the LPN supervises direct care staff and makes decisions about resident care needs during the shift, within the scope of clinical competence and facility policies.

Duties and Essential Functions
  1. Makes daily rounds on assigned units to observe, examine, or interview residents, verify information from ongoing shifts, monitor regulatory compliance, and determine staff assignments.
  2. In the absence of the Registered Nurse, develops and distributes resident care assignments to direct care staff, matching resident needs with staff skills and adjusting assignments as necessary during the shift.
  3. Directs daily assignments and evaluates the work of other caregivers, including CNAs, CMTs, Restorative Nursing Assistants, and Preceptor Nursing Assistants.
  4. Using independent judgment, makes recommendations regarding the care of residents, discipline, discharge, promotion, and other personnel matters.
Education, Licenses, and Certifications

Minimum requirements include:

  • Graduation from an accredited school for practical nursing.
  • Current and active license as a Licensed Practical Nurse in the state of Kentucky.
Preferred Qualifications

One year of experience providing direct care to residents in a long-term care setting is preferred.

Examples of these stresses include, but are not limited to, shift rotation, weekend and holiday duty, unusual or impaired behavior by residents, family reactions to having a loved one in the nursing home, death, and dying, oversight of state surveyors, ombudsmen, and federal officials, presence of consultants and attorneys, and variable involvement of medical staff.

While performing the duties of this job, the team member is occasionally required to sit, use hands to finger, handle, or feel objects, tools, or controls, reach with hands and arms, and talk or hear.

The team member frequently is required to stand and walk. The team member is occasionally required to climb or balance and stoop, kneel, crouch, or crawl. Occasional physical effort with light to medium objects. Occasionally required to lift objects of 10 to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and peripheral vision.

Frequently required to operate a computer and other equipment. Frequently required to use fine motor skills to handle and operate instruments/devices. Frequently required to lift and/or move items up to 50 pounds. Occasionally required to lift and/or move items up to 100 pounds. Constantly required to communicate with patients and other care team requiring talking and hearing. Occasionally required to position self in order to meet patient needs.
