Key Responsibilities:

- Provide skilled nursing services, including wound management, medication administration, and vital sign monitoring.
- Conduct regular home visits, assessing patient progress and adjusting care plans as needed.
- Foster strong relationships with patients, families, and caregivers, ensuring open communication and emotional support.
- Participate in interdisciplinary team meetings, contributing to the development of comprehensive care plans and addressing complex patient issues.
- Stay up-to-date with industry developments, best practices, and regulatory requirements, maintaining a high level of professional expertise.
Requirements:
- Current licensure as a registered nurse (RN) in the state of practice.
- Minimum 2 years of experience in hospice or palliative care, preferably with a focus on home-based care.
- Strong clinical skills, with ability to assess patient needs, prioritize interventions, and manage complex medical conditions.
- Excellent communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare professionals.
- Capacity to work independently, with minimal supervision, while maintaining strong ties to the interdisciplinary team.
Salary and Benefits:
