The estimated salary for this role is $203,400 - $299,500 per year. This job is eligible for a bonus incentive plan based on company and individual performance.

- Work collaboratively with other members of the care team to ensure that patients receive comprehensive and coordinated care.
- Maintain accurate and up-to-date medical records for all patients.
