We are looking for a skilled professional who can deliver high-quality patient care in a fast-paced environment. As a Primary Care Provider, you will be responsible for developing and executing person-centered care plans, working closely with a multidisciplinary team, and leveraging care navigators to maximize provider time and energy.
Key Responsibilities:
- Provide onsite primary care services to older adults living in Senior Living Communities
- Develop and execute detailed care plans for primary care patients
- Collaborate with Medical Director to identify and develop policies for emerging diseases
- Work with Care Navigators to provide monthly Chronic Care Management visits and care
- Lead regular huddles and provide leadership to the clinical team
- Identify opportunities for collaboration with senior living management and care teams to improve patient outcomes
Requirements:
- Family Medicine or Nurse Practitioner or Physician Assistant experience, preferably with geriatrics experience
- Ability to work in a fast-paced environment and adapt to changing circumstances
- Comfortable providing onsite clinical care in Marin County
- Strong orientation towards collaboration and teamwork
- Excellent written and verbal communication skills
- Ability to work with tech-enabled care delivery tools
