About the Role

We are seeking a skilled Nurse (Diabetes) - ESEP/MP to join our team at the Department of Health and Human Services in Gallup, NM. As an Outpatient Nurse in the Diabetes Program, you will be responsible for providing comprehensive nursing care to ambulatory patients.

Key Responsibilities
  • Independently evaluates patient status and patient's standard of care and revises necessary to meet the revisions in the medical care plan and patient's changing needs.
  • Participate in the development of nursing procedures and performance improvements standards for medical services.
  • Works with members of the Diabetes Team to maintain an accurate patient registry to follow up on patients that have not been seen within the past 12 months, and to follow up on patients who require intensive management.
Requirements
  • A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required.
  • Possession of a master's degree in professional nursing; or at least one full year of professional nursing experience (equivalent to the GS-7 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program.
