Senior Community Care Nurse Practitioner in Monroe County, NY

Key Responsibilities:
- Conduct comprehensive assessments and manage members through acute medical situations or exacerbations of chronic conditions.
- Manage members during short-term sub-acute rehabilitation episodes and ensure accurate and complete documentation, including ICD-10 conditions.
- Communicate and collaborate with the interdisciplinary care team, conduct advanced illness and advanced care planning conversations, and provide patients and caregivers with counseling and education.
Requirements:
- Education: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted license in the state where you reside, or ability to obtain by start date.
- Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions.
Preferred Qualifications:
- 1+ years of clinical experience in practice, preferably in a long-term care setting.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of Geriatrics and Chronic Illness, as well as Advanced Illness and end-of-life discussions.
- Proficient computer skills, including the ability to document medical information with written and electronic medical records.
Compensation and Benefits:
The salary range for this role is $88,000 to $173,200 annually, based on local labor markets, education, work experience, certifications, and other factors. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, you'll receive a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, subject to eligibility requirements.
