Job Summary

The Case Management Associate plays a crucial role in supporting the daily activities of SW/RN Case Managers. This position is responsible for assisting in the development and maintenance of systems to ensure seamless case management operations.

Key Responsibilities
  • Collaborate with SW/RN Case Managers to develop and implement effective discharge planning strategies.
  • Assist with utilization review activities to ensure optimal patient care and resource allocation.
  • Provide administrative support to the case management team, including data entry and record-keeping.
  • Participate in quality improvement initiatives to enhance case management processes and outcomes.
Requirements
  • 3 years of experience in patient care or a related healthcare field is preferred.
  • Previous experience in case management, discharge planning, or utilization management is strongly preferred.
  • Bilingual skills, particularly in Spanish, are highly valued.
  • Must demonstrate commitment to STHS's Compliance Program and Code of Conduct.
Skills and Qualifications
  • Effective communication and interpersonal skills.
  • Ability to maintain confidentiality and adapt to changing priorities.
  • Knowledge of computers, Microsoft Office applications, and electronic medical records.
Education and Licensure
  • High school graduate or GED is required.
  • Preferred 2 years of health-related work experience.
