**Job Summary:**

The Clinical Operations Coordinator will be responsible for the daily administrative clinical operations of the hospice in collaboration with the Clinical Manager. This role will coordinate the IDG schedules, monitor documentation and clinical productivity, participate in the morning stand-up calls, and monitor quality metrics.

**Key Responsibilities:**

  • **Coordinate Daily Administrative Clinical Functions:**
  • Work under the direction of the Clinical Manager to oversee the coordination of daily administrative clinical functions for hospice services while demonstrating ethical and professional conduct.
  • **Monitor Quality Components:**
  • Monitor quality components of hospice services, including note completion, schedules, visit frequencies, complaint tracking, etc.
  • **Participate in Data Collection and Reporting:**
  • Participate in regular data collection and reporting as directed by the Clinical Manager.
  • **Recruitment, Retention, and Training:**
  • Participate in recruitment, retention, and training of IDG members.
  • **Collaboration and Communication:**
  • Work in collaboration with the Clinical Manager on on-call schedules, weekend schedules, and per diem staffing.
  • **Documentation and Compliance:**
  • Appropriately document clinical and administrative work.
  • Monitor documentation of incidents and medication errors and report to Hospice leadership.
  • Ensure the use of utmost care in protecting the confidentiality of client records under State and Federal laws.
  • Ensure compliance with all hospice protocols.
  • Present changes, improvements, or additions to protocols or procedures to the Hospice Clinical Manager.
  • **Additional Responsibilities:**
  • Perform all other duties as required.
  • Adhere to all Policies and Procedures.
  • Conduct self in a manner that represents our values at all times.
  • Maintain a positive and respectful attitude with all work-related contacts.
  • Communicate regularly with the supervisor about departmental concerns.
  • Consistently report to work on time, prepared to perform the duties of the position.
  • Meet productivity standards and perform duties as workload necessitates.

**Qualifications:**

  • High school graduate or its equivalency.
  • Two years of hospice or home health experience as an LPN.
  • License Practical Nurse or related field preferred.
  • Supervisory experience required.
  • Must possess good communication skills, written and verbal, and the ability to work well with a variety of people.
  • Ability to relate well to patients, families, co-workers, and physicians.
