Key Responsibilities:

  • Participates in patient assessments by gathering both subjective and objective information.
  • Works alongside the Registered Nurse to execute the patient care plan.
  • Facilitates communication with patients, families, and the healthcare team to ensure coordinated care.
  • Oversees and assesses care provided by non-licensed staff members.
  • Records nursing assessments, interventions, and evaluations of patient responses.
  • Recognizes potential emergencies and takes appropriate action.
  • Provides educational materials to patients and families under the guidance of the RN.
  • Engages in the ongoing development and adjustment of the care plan based on patient feedback.
  • Maintains a safe environment by following hospital protocols for isolation, restraint use, fall prevention, blood transfusions, and medication administration.
  • Participates in mandatory skills validation, staff meetings, and continuing education as required.
  • Assists in orienting new staff members.
  • Complies with all relevant laws and ethical standards.
  • Performs additional duties as needed based on workload and staffing requirements.

Qualifications:

Education: Completion of an accredited LPN program.

Licensure: Valid LPN license in Indiana. BLS certification required upon hire or within 60 days.

Experience: No prior experience necessary beyond educational training.

Additional Requirements:

Possesses the knowledge and skills to follow care plans appropriate for the patient’s age.

Work hours and shifts may vary based on departmental needs.
