We are seeking an experienced Advanced Practice Provider to join our Abdominal Transplant Service team at Trinity Health. As a key member of our multidisciplinary team, you will provide high-quality patient care and coordinate services to support the transplant program.
Key Responsibilities- Coordinate patient care and collaborate with attending physicians, resident house staff, and other team members to ensure seamless transitions and optimal patient outcomes.
- Design and evaluate individualized treatment plans, taking into account patient needs and preferences.
- Provide education to patients and families on transplant care and related healthcare decisions.
- Participate in quality improvement processes and remain compliant with regulatory bodies.
- Master's degree as an Acute Care Nurse Practitioner or Physician Assistant.
- At least 1 year of inpatient APP experience, with 2-5 years preferred.
- CPR, ACLS, and Illinois Registered Nurse License, with ability to qualify for Illinois and DEA prescribing numbers.
