Job Title: RN Long Term Care

At Amergis, we are seeking a skilled and compassionate Registered Nurse to join our team in a Long Term Care setting. As a RN Long Term Care, you will be responsible for providing high-quality patient care to individuals in a Long Term Care and/or Assisted Living Facility.

Key Responsibilities:
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility
  • Perform focused interviews to identify specific patient needs
  • Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition
  • Collect, analyze, and interpret data and information from healthcare team members and document actual and/or potential nursing diagnoses
  • Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions as well as responses to interventions
  • Perform interventions according to identified priorities, plan of care, and facility policies and patient outcome standards
  • Provide a safe, comfortable, and therapeutic patient environment
  • Initiate and evaluate patient and/or family education
  • Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit, including establishing priorities
  • Monitor safety and effectiveness of the environment and equipment
  • Participate in activities to facilitate good interpersonal communications with the chosen facility
  • Support professional practice by observing the following: uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4) Joint Commission, state and local laws; 5) Patient's Bill of Rights; and 6) Amergis Healthcare, nursing and client hospital policies
Requirements:
  • Current Registered Nurse License for the state in which the nurse practices
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB Questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state, and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
  • Computer proficiency required
  • Must be at least 18 years of age
