Job Summary

We are seeking a skilled Care Coordinator to join our team in Maine. As a member of our team, you will be responsible for providing targeted case management and behavioral health home services to clients in their homes. This will involve conducting home visits, assessing client strengths and needs, and linking them to community resources.

Key Responsibilities:

  • Conduct home visits to assess client strengths and needs
  • Link clients to community resources and develop individualized treatment plans
  • Collaborate with educational, vocational, medical, and behavioral health providers to provide wrap-around services
  • Meet with clients and families regularly to engage them in their progress and success

Requirements

To be considered for this role, you must have a Bachelor's degree in any field. Experience or a degree in social, behavioral, or health services is preferred but not required. Additionally, MHRT/C certification is a plus.

Compensation and Benefits

As a full-time employee at Clarvida - Maine, you can expect a competitive compensation package, including an estimated annual salary of $52,000, as well as benefits such as paid vacation days, separate sick leave, paid holidays, and more.
