About the Role:- Provide comprehensive patient care in an independent setting, including assessing, diagnosing, and implementing complete care plans.
- Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
- Collaborate with multidisciplinary teams to identify and address patient needs, and develop individualized care plans.
- Deliver education and training to patients and caregivers on home hemodialysis, including equipment use, medication management, and lifestyle modifications.
- Monitor patient progress, identify potential complications, and adjust care plans as needed.
- Participate in quality improvement initiatives to enhance patient outcomes and program performance.
