We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at SB Clinical Practice Management. The successful candidate will be responsible for coordinating the daily operations of our front information desk, ensuring efficient workflow, patient care, and assisting with patient transportation.
Key Responsibilities- Coordinate Patient Flow: Greet all scheduled patients upon arrival, assess telephone calls, and refer urgent medical queries to physicians or nurses immediately.
- Transportation Assistance: Utilize Staxi Chairs to assist with transporting patients from their vehicle to their appointment and back.
- Patient Tracking: Track all new patient inquiries, including patient demographics, appointment date, and provider that appointment was scheduled with.
- Follow-up and Communication: Follow up with newly scheduled patients to assess patient experience and perform all necessary receptionist duties, including answering, screening, and routing of phone calls.
- Facilitate Patient Care: Facilitate patient flow by notifying providers of patients' arrival, delays, or backlogs and communicate with other staff and/or patients as necessary.
- Electronic Medical Record Management: Utilize Electronic Medical Record (EMR) and/or other systems for appointment navigation.
- Visitor Tracking: Track all visitors in the e-log for contact tracing as required by the CDC/DOH.
- Reception Area Maintenance: Maintain clean and organized reception area.
- Additional Responsibilities: Provide additional coverage for evening hours and weekends, as needed, and coordinate with building security and management to ensure patient safety.
- Education: High School diploma/GED.
- Communication and Organizational Skills: Strong organizational and communication skills (both verbal and written).
- Typing and Telephone Etiquette: Excellent typing skills and friendly telephone etiquette.
- Medical Office Administration: Graduate of an accredited Medical Office Administration program.
- Receptionist Experience: 6 months experience working as a Receptionist.
- Clerical or Customer Service Experience: 1 year of clerical or customer service experience.
- EMR and Practice Management System Experience: Experience using an EMR and a practice management system.
- Bilingual: Bilingual in Spanish.
