Behavioral Health Case Manager

We are seeking a highly skilled and compassionate Behavioral Health Case Manager to join our team at Consejo Counseling and Referral Service. As a key member of our Behavioral Health Programs, you will work in partnership with clients and the Behavior Health team to provide responsive, culturally sensitive, and comprehensive services.
Key Responsibilities- Maintain a caseload and provide a minimum of four direct client service hours per day.
- Provide advocacy, outreach, information, and referral services to clients in need of case management services.
- Coordinate services with Behavior Health staff and other service providers to ensure optimal client care.
- Assess crises and provide crisis interventions when necessary.
- Establish liaisons to access community services, such as medical care, housing, and food.
- Provide education, outreach, and information in the community as needed.
- Provide transportation for SUD and MH clients to groups as needed.
- Provide family support and support for others of significant importance to the client.
- Serve as a liaison between the client and multiple systems utilized by the client.
- Bachelor's degree or AA with 2 or more years of experience.
- Obtain the proper credentialing through the state of Washington as Agency Affiliated.
- Experience in major specialization in social, behavioral, or human services or human-related fields or with related volunteer experience comparable to two years of full-time work in social, behavioral, or human services or related fields.
- Proficient in speaking, reading, and writing both English and Spanish.
- Experience working with a mental health population preferred.
This full-time in-person therapy position works a typical schedule of 8 AM - 5 PM, Monday to Friday.
Salary Range$27.88/hr. depending on experience, qualifications, and credentials.
