Job Summary

We are seeking a skilled and compassionate Pediatric Home Health Nurse to join our team. As a Pediatric Home Health Nurse, you will provide high-quality patient care and support to children and families in the comfort of their own homes. Responsibilities

  • Provide skilled nursing care to pediatric patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Provide education and support to patients and families on disease management, medication adherence, and healthy lifestyle habits.

Requirements

  • Current licensure as a Registered Nurse (RN) or Licensed Vocational Nurse (LVN) in the state of Texas.
  • Valid CPR certification.
  • Experience working with pediatric patients and families, preferably in a home health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering exceptional patient care and support.
