Job Overview

Position: Evening Hospice RN Case Manager

Employment Type: Per Diem

Location: All Territories, ME, US

Key Responsibilities:

  • Deliver proficient nursing care to patients in their residences, adhering to the physician's prescribed care plan.
  • Collaborate with patients and families to formulate care plans with achievable and measurable outcomes.
  • Educate patients and advocate for their rights and responsibilities to facilitate informed choices regarding their care.
  • Evaluate and address the comprehensive health care needs of patients.
  • Exhibit proficiency in both assessment and technical nursing skills.
  • Utilize expertise in pain management and symptom control.

Qualifications:

  • Active R.N. license in the State of Maine.
  • ANA certification in Home Health or Community Health Nursing is preferred.
  • Current CPR certification is mandatory.
  • A minimum of one year of recent clinical nursing experience is required.
  • Access to a reliable vehicle with valid insurance.
