We are seeking a skilled and compassionate Home Health Nurse to join our team at Androscoggin Home Healthcare + Hospice. As a Home Health Nurse, you will provide high-quality nursing care to patients in their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.
Key Responsibilities:
- Provide skilled nursing services to patients in their homes, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Conduct thorough assessments of patients' physical, emotional, and social needs, identifying areas for improvement and developing strategies for addressing these needs.
- Provide education and support to patients and families on disease management, medication adherence, and other health-related topics.
- Participate in ongoing quality improvement initiatives to ensure the highest level of care and service delivery.
Requirements:
- Current RN license in the State of Maine.
- ANA certification in Home Health or Community Health Nursing encouraged.
- Current CPR card required.
- One year of recent clinical nursing experience required.
- A reliable car with insurance.
