  • Conduct thorough needs assessments to identify gaps in services, barriers to care, and opportunities for improvement within current state.
  • Collaborate with interdisciplinary team members to ensure programs are woven together and have seamless operations.

Qualifications
  • Bachelor's degree in nursing (BSN)
  • 5 years of clinical experience in healthcare settings, with a focus on care coordination, population health management, or related areas in an integrated healthcare delivery system.
  • Registered Nurse (RN) license in North Carolina.
